1st Touch Foundation Referral Process

1. Referral Comes In:
Intake call or receipt of Referral Form via mail, e-mail, or fax

2. Referral Confirmed:
Family being referred to The 1st Touch Foundation is contacted to confirm
their interest Project 3000

3. If Confirmed:
A Welcome Kit is sent to the family
(Parent Welcome Kit, Project 3000 Kit, Individual Kids’ Packages included)

4. If NOT Confirmed:
Call is made to the person who made the referral

5. Enter Into Database:
Referral information is entered into The 1st Touch Foundation’s confidential
data base and coded so that we can track each person’s status (Active,
Alumni, Opt Out, Lost – New Address Needed, Bereaved, etc.)

6. Ongoing Mailings:
• Kids with LCA (birth – 18yrs.) will be entered into our Little Believers
Program. Twice a year they will receive age-appropriate 1st Touch
Foundation “We’re Thinking of You” Packages.
• Sighted siblings (birth-18yrs.) will be entered into our STARs
Program. Twice a year they will receive age-appropriate 1st Touch
Foundation “We’re Thinking of You” Packages.
• Families will receive The 1st Touch Foundation’s Quarterly
Newsletter, Believe.

7. Annual Follow-Up to The 1st Touch Foundation:
In October, we send an evaluation survey to each child and sibling in our
program, and a survey to each parent

8. Maintenance:
• The 1st Touch Foundation must be notified if the family has moved.
• Families can “opt out” of being a 1st Touch Foundation Member at any
time.
• When a child turns 19, they are moved into The 1st Touch Foundation’s
“Alumni” status. The “We’re Thinking of You” packages will be
replaced with the Quarterly News Letter, Believe.
